To file a complaint about a specific food establishment, you must first identify it by name or location.  Search for an establishment by license type, or, if you know it, the establishment’s license number.  The more information you provide, the more specific the search results will be.  Once you have entered as much information as possible, click the “search” button to generate a results page.

The results page displays a list of all licensed establishments meeting your search criteria.  Once you have clicked on an establishment name, a form will appear which will allow you to enter the details of your complaint.
